Results of an Effort to Curtail the Number of "Unacceptable" Medications in the Elderly Population via Drug Regimen Review

In this study, consultant pharmacists were successful in reducing or discontinuing the use of 43% of unacceptable medications identified. After review by the consultant pharmacists, it was determined that 23% of the medication orders identified by the initial computer search had a potentially legitimate reason for use.

Monitoring Total Parenteral Nutrition Therapy in the Elderly

A multidisciplinary approach to parenteral nutrition monitoring and complication prevention is important, particularly when the patient is in transition from acute-care to long-term or home care. Measuring effectiveness of the therapy, as well as preventing complications such as catheter infection, thrombosis, and refeeding syndrome, are the keys to optimal patient outcomes.
