Post Acute Care Payment Reform Demonstration

The Deficit Reduction Act of 2005 directed the Centers for Medicare & Medicaid Services to develop a Post Acute Care (PAC) Payment Reform Demonstration.  The goal of this initiative is to standardize patient assessment information from PAC settings and to use these data to guide payment policy in the Medicare program.  As part of this initiative, a draft universal patient assessment tool has been developed for use upon hospital discharge of Medicare beneficiaries, and admission to nursing facility, home health agencies, inpatient rehabilitation facilities, and long term care hospitals.  This tool is intended to eventually replace setting specific assessment tools, such as the Minimum Data Set in nursing facilities.  Comments are being received by CMS on the draft assessment tool through September 25, 2007.  ASCP will be submitting comments.
