Addressing Social Determinants of Health for Geriatric Patients During Transitions of Care

The Centers for Medicare and Medicaid Services (CMS) describes transitions of care as the movement of patients between practice settings, providers, and levels of care. At each healthcare transition, patients are vulnerable to ineffective or duplicative care, medication errors, and adverse health outcomes due to lack of standardized processes, ineffective hand-off communication, and poor continuity of care. Geriatric patients have unique challenges during healthcare transitions due to pathophysiology of aging, chronic disease state management and multiple comorbidities, and polypharmacy. Additionally, social determinants of health (SDOH) may impact access to healthcare resources and adherence to medication therapies. Pharmacist interventions during transition of care can help improve health outcomes by supporting safe and effective transitions of care. Assessing and addressing SDOH and access barriers are foundational to optimizing patient care for geriatric patients.

Learning Objectives

At the conclusion of this application-based activity, participants should be able to:

  • Examine unique challenges geriatric patients encounter during transitions of care
  • Interpret the impact of social determinants of health (SDOH) during transitions of care
  • Employ pharmacist interventions to address SDOH and optimize health outcomes during healthcare transitions

How to Access Content

  1. Select “launch” to open the course. Click through the menu items in sequence to move through the course process to claim credit. You must complete the course in its entirety to claim credit

  2. Post-test must be completed with minimum score of 75% before moving on to the evaluation.

  3. All courses contain an evaluation that must be completed for full CPE credit. No exceptions or extensions permitted.

  4. Once you have completed the course evaluation, please confirm proper submission by checking your CPE monitor and/or BPS portal (BCGP pharmacists) within 24-48 hours of course completion.

BCGP Pharmacists: To ensure successful transfer of BCGP credit, your ASCP profile must be updated with your correct BCGP Credential Number and BPS-IND# prior to course completion. *Please note, any edit/updates made to your BCGP credential information in your ASCP profile after course completion requires a re-push, please submit your completed transcript to education@ascp.com for a manual upload.

CPE & BCGP Credit: You must complete the post-test and evaluation by the expiration date to receive credit. You have three attempts to achieve a 75% or higher on the 8-question post-test. ASCP reports CPE attainment to ACPE and NABP via the CPE Monitor system. The course should be posted within 24-48 hours of evaluation completion. The participant is responsible for providing their correct date of birth (MMDD format) and NABP e-Profile ID on their member or guest profile on the ASCP Website.

This activity must be completed by the expiration date. No exceptions or extensions permitted.
