Fostering effective and safe medication use is essential to promoting positive health outcomes and improving life quality for older persons.
Focusing on the practice of senior care pharmacy and advancing the profession, the ASCP Foundation strives to improve the health and well-being of older persons through appropriate, effective, and safe medication use.
To achieve this, the Foundation engages in educational and research initiatives that include:
A landmark three-phase research initiative administered by the ASCP Foundation to demonstrate the impact of consultant pharmacist services on patient outcomes and health care costs.
A three-year research project focusing on improving patient safety by reducing serious, preventable adverse drug events through the use of the Geriatric Risk Assessment MedGuide (GRAM) software.
Guidelines that provide pharmacists and pharmacies with recommendations for making important medication information accessible for patients with vision loss, and serve as a resource for persons with vision loss and organizations serving them.
This award is bestowed annually by the ASCP Foundation Board of Trustees to a pharmacy organization that is an exemplary provider of pharmacy services to residents and patients dwelling in the post-acute, long-term care, assisted living, or community settings. Meet this year's recipient.
