To help older adults and those who care for them navigate the complexities of managing medications and how they affect what matters to them, their mentation and mobility, this initiative will incorporate the expertise of long-term care pharmacies and pharmacists who specialize in the care of older adults into the Age-Friendly Health Systems movement. Funding will support the American Society of Consultant Pharmacists (ASCP) to educate and foster pharmacy champions in the Age-Friendly Health Systems 4Ms Framework. ASCP will partner with the Peter Lamy Center on Drug Therapy and Aging to champion age-friendly care and optimal medication management. The project team will guide at least 250 LTC pharmacies to be recognized as achieving age-friendly national recognition through a pharmacy-based credential or badge once they have submitted a description of how the 4Ms are being operationalized and approved by ASCP guidelines. The 4MsWhat Matters Know and align care with each older adult’s specific health outcome goals and care preferences including, but not limited to, end-of-life care, and across settings of care Medication If medication is necessary, use age- friendly medication that does not interfere with What Matters to the older adult, Mobility, or Mentation across settings of care Mentation Prevent, identify, treat, and manage delirium across settings of care Mobility Ensure that each older adult moves safely every day to maintain function and do What Matters PartnersThe John A.
