Many discussions related to accountable care organizations (ACOs) and other emerging models of care focus on hospitals and physicians, but rarely address pharmacists or medication management. This one-day continuing pharmacy education (CPE) program focused on pharmacists’ involvement in ACOs and other care models, where we stand, what to expect next, and how to become a part of this growing trend.
The program looked at pharmacy services from an operational perspective, including the role of a long-term care facility as well as the medication management services a pharmacist could provide as part of the care team. Attendees received perspective from professionals working successfully in these new models, and use the speaker's experiences to help formulate an action plan for their own involvement.
