Engaging Pharmacists in Emerging Models of Care

Many discussions related to accountable care organizations (ACOs) and other emerging models of care focus on hospitals and physicians, but rarely address pharmacists or medication management. This one-day continuing pharmacy education (CPE) program focused on pharmacists’ involvement in ACOs and other care models, where we stand, what to expect next, and how to become a part of this growing trend.

The program looked at pharmacy services from an operational perspective, including the role of a long-term care facility as well as the medication management services a pharmacist could provide as part of the care team.  Attendees got perspective from professionals working successfully in these new models, and used the speaker's experiences to help formulate an action plan for their own involvement.

  1. Assess new opportunities presented by the Affordable Care Act (ACA) for providers to diversify and expand the services provided to beneficiaries.

Target Audience
This knowledge-based activity is designed for pharmacists who have an interest in or are unfamiliar with how LTC pharmacist and consultant pharmacist services might work in conjunction with ACOs and other emerging models of care and how they might begin to position themselves for increased involvement in this arena.

Requirements to Obtain CE
To receive continuing pharmacy education (CPE) credit, participants must attend the activity in its entirety and evaluate the activity on the ASCP Professional Development CEnter. No partial credit will be given. To access the evaluation, participants must enter the CPE code for the educational activity attended. The CPE codes will be announced during the program introduction. ASCP reports CPE attainment to ACPE and NABP via the CPE Monitor system. It is the responsibility of the participant to provide their correct MMDD of birth and unique CPE Monitor identifier number on their member or guest profile of the ASCP Website.

Continuing Pharmacy Education
The American Society of Consultant Pharmacists (ASCP) is accredited by the Accreditation Council for Pharmacy Education (ACPE) as a provider of continuing pharmacy education. This knowledge-based activity (0203-0000-14-030-L04-P) is approved for 6.0 contact hours.  Pending approval from the FL Board of Pharmacy and Alabama State Board of Pharmacy.
